APPENDIX G - Application Forms
The application forms needed for USATF course certification are all provided in a single file named “appforms.pdf” which you can download from this page in Adobe PDF format. The forms contained in this file are as follows (Note: If you’ve arrived at this page without reading our Course Measurement Manual, please be sure to study the manual before using any of these forms):
To read and print these forms, you may download the free Adobe Reader software if you don’t have it already.
Our appforms.pdf file is downloaded using the link below. Depending on how your browser is configured, selecting that link normally (e.g., by left-clicking) may open the PDF file in your web browser. If that happens, you may then click a button immediately above the displayed form to save the file to your hard drive. Alternatively, you may download the file to your hard drive directly (without displaying it in your web browser) by right-clicking (Windows) or control-clicking (Mac) on the link, and then selecting an item from the resulting popup menu. Either way, we recommend saving the appforms.pdf file, so you can easily print application forms whenever needed.
Note: This PDF file is "Typewriter enabled," meaning that you can "type" anywhere on any of the forms using the Typewriter tool in Adobe Reader 7.0 or greater, and you can save your filled-out forms in electronic form.
You’ll probably have to include a processing fee when sending the completed application forms to your regional certifier. The exact amounts of these fees vary from state to state. Therefore, check with your certifier before sending in the application forms.
